Dear Parents

Music Tuition Academic Year 2008/2009

We need to book our lessons for the next academic year, September 2008 onwards.

If you require instrumental tuition for your child please complete the form attached and return by Friday 27th June 2008.

If your child is currently learning an instrument this form will still need to be completed.

We will provide tuition in the following instruments:

Recorder	(Year 1 - Year 5)
Brass	(Year 3 - Year 5)
Clarinet	(Year 3 - Year 5)
Flute	(Year 3 - Year 5)

Tuition fees will be:-

Recorder	£2.00 per lesson (Group lesson)
Brass/Clarinet & Flute	£4.00 per lesson (paired lesson)
Individual Lesson	£8.00 per lesson (20 minutes)
Instrumental hire (if required)	£20 per term

Tuition fees may be subject to slight alteration depending on numbers and costs.

All tuition/rental fees need to be paid before your child has their first lesson.

Please be aware that in booking lessons/rental on this form you are committing yourself to paying for the service provided. Notice of a half term must be given if your child decides to cease lessons.

We hope as many children as possible would like to take this opportunity of learning an instrument. If you have any queries please come in and see me.
